UNNI'S INFANTILE DIABETES
When Unni Maya was born last year in the rural outskirts of Keezhukkunu, her parents were overjoyed, their first baby had been born premature, and died but Unni had been born after a full term weighing 2.5kg. Her postnatal period was uneventful, but after six months she started vomiting and developed a low-grade fever.
After Unni was hospitalized, doctors informed her parents that she had Insulin-Dependent Diabetes Mellitus (IDDM) with Diabetic Ketoacidosis (DKA), and she was put on insulin therapy. Unfortunately, she was also found to have Bilateral Congenital Cataracts, so she was referred to the Aravind Eye Hospital in Madurai by her paediatrician for further management.
When they arrived at Aravind, doctor in the paediatric clinic determined that the baby was able to follow light with both eyes open, but a look at the anterior segment of her eye revealed dense cataracts that made Fundus examinations impossible Based on the history of IDDM, dense total cataract and negative blood investigation, a diagnosis of congenital metabolic cataract due to infantile diabetes was considered.
Congenital Diabetes Cataracts are a relatively rare occurance; Cataracts due to Diabetes can be either due to senility or what are called "true" Diabetes Cataracts. These types of Cataracts (also known as Snowflake Cataracts) tend to occur in parents who have severe Diabetes (yet who are relatively healthy, and in whom water balance is maintaining well).
These may appear at an earlier age in cases of IDDM. In adolescents between the ages of 7 and 16 years of age they become more common, but the condition is rare in adults.
Doctor in the Paediatric Clinic decided that the baby needed and Extracapsular Cataract Extraction with Posterior Polar (PPC) with AV in both eyes after a month. The surgery was successful.
The operative treatment of Cataracts in patients with Diabetes can be more difficult than those in most patients; still, the presence of Diabetes is not in itself contraindicated for dealing with a patient's Cataracts as the doctor would normally.
Two difficult complications that doctors need to watch out for are the possiblity of haemorrhage and subsequent development of the kinds of infections which often results from uncontrolled Diabetes. The patient's blood sugar needs to be kept under control with adequate dosage of Insulin, but the doctor needs to be careful not to push insulin in large doses immediately before the operation since there is evidence that this increases the liability to haemorrhage. Fortunately in the case of Unni, intraoperative and postoperative period was uneventful.
While Unni will have to be on Insulin for the rest of her life (and face other challenges due to her Diabetes), fortunately her treatment at Aravind has preserved her sight. We wish she well!
